Cardiovascular

Cardiovascular disease is an umbrella term for over 60 different conditions that affect the heart and blood vessels. Cardiovascular disease treatment is tailored to each patient's needs and some possible treatment plans can include lifestyle changes, medication, surgeries, or rehabilitation. As with any health condition, prevention and a healthy lifestyle are encouraged to decrease any possibility of a cardiovascular disease or condition from developing. We provide comprehensive cardiac evaluation and risk factor assessment. This includes identification of personal factors that increase the chance of developing heart disease, a cardiac exam with EKG, an estimation of heart disease risk over 10 years, and a complete exercise stress test. If an Echocardiogram is required , this test can be performed in our facility. At Cross County Family Practice, our Family Physician is dedicated to partnering with each patient to provide the utmost quality care and treatment plan that is best suited to every need.

Patient-Centered Medical Home

The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care. NCQA Patient-Centered Medical Home (PCMH) Recognition is the most widely-used way to transform primary care practices into medical homes. Learn More >>
